Formal registration to the practice which allows the staff to administer to your insurance information, determine benefits and speed up the process on the day of your first appointment with Dr. Lee
Transfer of Medical Records to Dr. Lee's Office If you have records that Dr. Lee needs to review and they are at another doctor's office you will need to fill this form out and send it to your old doctor. Then they will send a copy of your medical chart to Dr. Lee directly.
